CONFLICT OF INTEREST INFORMATION

As per the National Constitution of Lambda Sigma Upsilon Latino Fraternity, Incorporated No member or associate member can be affiliated with or seek membership within any nationally recognized fraternity, or social fellowship, whether it be Greek-lettered or non-Greek lettered, and petition for membership to Lambda Sigma Upsilon with the exception of Freemasonry and or its appending bodies.

*Please note a copy of the letter of renunciation of membership must be submitted to the Cacique of the chapter at least one week prior to the beginning of the New Member Education Process. Please be aware that you are not allowed to disclose in whole or part any of the rituals, ceremonies and activities of the New Member Education Process of Lambda Sigma Upsilon, to any non-member of the fraternity, and doing so, is a violation of the Confidentiality Agreement of Lambda Sigma Upsilon, thus making you legally liable under the legal and civil penalties as stated in the agreement.

CERTIFICATION AND ACADEMIC AGREEMENT

I certify that all the aforementioned information is true to the best of my knowledge, and I am aware that to knowingly give misleading or false information can/will terminate my consideration for, or terminate my participation in the New Member Education Process (NMEP) of Lambda Sigma Upsilon Latino Fraternity, Incorporated. I also agree that while I participate in the New Member Education Process (NMEP) of Lambda Sigma Upsilon Latino Fraternity, Inc. will maintain a minimum passing grade of no less than a C in all my classes. I understand Lambda Sigma Upsilon has the right to receive my grades and to monitor the progress of my academic work. Furthermore I understand should I receive a failing grade on any test, report or any other class requirement I agree to remove myself from the NMEP unless a passing grade can be achieved in the continuum. The undersigned hereby understands the terms stated above and agrees to comply with the aforementioned declaration. This document and its contents are considered null and void without the required signatures and supporting documentation. Please note that ALL information will be held to the strictest confidentiality, as required by law. Saying yes to any or all of the aforementioned questions will not disqualify you from participating in the New Member Education Process of Lambda Sigma Upsilon, but failure to knowingly answer the questions honestly will lead to termination or consideration to participate in the NMEP of Lambda Sigma Upsilon Latino Fraternity, Inc. Associate Member Confidentiality Agreement I do hereby declare should I be selected to participate in the New Member Education Process of Lambda Sigma Upsilon Latino Fraternity, Inc. I will not divulge any information, which shall be entrusted to me, to anyone who is not a fully initiated and recognized member of Lambda Sigma Upsilon. I am aware that all information entrusted to me is confidential and may be but not limited to: fraternal history, financial information, rituals, ceremonies, etc. this also includes written information and or all documents, or other modes of communication (i.e. verbal, visual) pertaining to the fraternity. I am also aware that I am not to photocopy, send via electronic or regular mail, telephone or any other mode of communication any documents or information deemed confidential by the fraternity to any one not a fully initiated and recognized member of Lambda Sigma Upsilon. Furthermore, I am aware that the Associate Member Application is a legally binding contract between Lambda Sigma Upsilon Latino Fraternity, Incorporated and I. It’s recognized by regional and national officers as well as chapters. This document will be considered as such by the fraternity and or any agents acting on the fraternity’s behalf. Failure to comply with the aforementioned can/will subject me to consequences of Breach of Contract and all penalties under such, thus making me liable to all legal & or civil actions against me by Lambda Sigma Upsilon, its officers and agents acting on behalf of the fraternity, its assets or interests. I also acknowledge the fact that this document is legally binding whether I become a fully initiated and recognized member or not. In conclusion, should I discontinue the New Member Education Process (NMEP) before becoming fully initiated and recognized member of the fraternity, I will return to the Induction Officer (Cacique) no later than 24 hours after discontinuing the NMEP, any uniform and all documentation and written information pertaining to and about Lambda Sigma Upsilon given to me during the NMEP. The undersigned hereby understands the terms stated above and agrees to comply with the aforementioned declaration. Associate Member Intent to Collect Agreement I am aware that Lambda Sigma Upsilon Latino Fraternity, Inc. has the right to, and will collect any outstanding monies owed the fraternity, or any of its recognized members or officers to offset the cost of my participation in the New Member Education Process (NMEP). My name below acknowledges my willingness to pay the dues “fees” of the (NMEP), I am aware and do consent with the collection of these fees regardless of becoming a fully initiated and recognized member of the fraternity or not, whether the termination of my participation in the (NMEP) is determined by me, the fraternity or any of its recognized officers.

If you have any concerns please reach out to the National Director of New Member Education [email protected] Please note that the monies paid to the fraternity are NON- REFUNDABLE, if the fraternity has already purchased the items needed for your participation in the (NMEP), in the event that you have not started your initiation into the fraternity you will be reimbursed all money you gave to the chapter, with the exception of $425 which will be used to cover application processing fees. By completing this application you have made your final decision to undergo the new member education program this semester and understand $425 is NON -REFUNDABLE.
